Home Care and Hospice
Home care services
are an essential part of a
patient’s recovery. This
service allows patients to
receive health care services
in their home. Services
offered include:
• Skilled Nursing
Visits
• Physical Therapy
• Occupational
Therapy
• Speech and
Language Therapy
• Medical Social
Work Services
• Certified Nurses
Aides and Homemakers
Home health care
services are
reimbursable through
Medicare, Medicaid,
worker’s compensation
and many health
insurance programs. Our
program is certified by
Medicare.
Hospice of the Estes
Valley offers a
multidisciplinary team
approach to working with
individuals and
families. Services
available include those
above and in addition:
• Pain Management
• Chaplain Services
• Volunteer Services
The Hospice program has
contracts with local
pharmacies and medical
equipment suppliers in
town to meet other
needs. As an additional
service, bereavement
counseling is available
for up to twelve months
after family members
have lost a loved one.
